The Nurses team will be in attendance on 26th November to offer all children from Reception to Year 6 a free nasal flu vaccine. Ahead of this we have been asked to send consent forms for completion. A copy is below.
In view of the importance of this consent, we will also be sending paper copies in bags today, but for Covid security, we ask that children return them and place into a box in the classroom where the forms can be held until the nurses need them. Please do not hand these to staff or bring to the office.
Thanks in advance for your support with this, we would ask that you return the consent form even if you take the decision to decline the offer.
